State of the Profession Early in the week our President spoke before our congress and exercised his obligation to outline the state of our nation. Goals met and challenges that lie before us. Which areas need a boost while others better suppressed. Budgets outlines and world concerns rounded off the due diligence that maintains a nation. Depending on which side of the political aisle you sat, the state of the union address was a masterpiece or a master flop. No need to fret, the nation will carry on. How would our state of the profession sound? My fellow chiropractors, I stand before this august body of men and women dedicated to the health and wellness of mankind. Our forefathers and mothers planting the seeds of alternative healthcare made our profession prominent amongst the many truly humble us today. We shall not forget their struggles with insurance and medical providers and with corporate and community ignorance. Perseverance and belief in a common goal reduced the spiral of medical failures and enhanced a closer look at deductive reasoning and conservative chiropractic health care. With a unified voice in every state house of this glorious nation, chiropractors stood shoulder to shoulder demanding respect and equitable fees for service rendered smack in the face of antiquated thinking thus changing the mind and soul of those suited to make legislative change. Patients felt a part of a healthy movement and massed with their doctors and staff demanding results and accepting no for an answer. They, along with friends and family of all ages, spoke to their representatives outlining their need for proactive care within all stages of the family lifecycle. Pregnant patients discussed using chiropractic throughout their pregnancy and how it helped alleviate the physical changes towards term. Young parents described their enhanced physical responsibilities and the rigors of toddlers readying to be out and about. Their discussion met with agreement as parent after parent agreed that preventive structural care kept their youngsters whole and happy. Teenagers spoke of peer pressure and the mechanical trials their body was going through. How better to monitor the physical challengers found in gymnastics, ball sports of all kinds and the like. Which healthcare professional matched adjustments and tweaking the body towards health and wellness and away from sickness and disease? Adults approached their colleagues at work and in houses of worship collectively demanding cost effective ergonomic and health and wellness workshops be in place and encouraged the chiropractic point of view. Local chiropractors with programs in hand met the demand. Seniors took to the floor feeling obligated to extend their appreciation for chiropractic keeping them young, aging gracefully and in safety. They demanded increased Medicare coverage with a broad expansion of services. Doctors, as I conclude our state of the profession, let me warn you that these words were but a dream. We all know how divisive and competitive we are. Seems we forget that there are no lack of patients just lack of educated people. Let us not forget that the smallest hamlet has gathering places with a common need for our care. Whether you are a mixer, straight or blender our goals are the same. Reduce pain, remove interference and let the body carry on. We stand tall and proud that our schools are graduating students able to cope with the demands of the day. Doctors both young and old, need go no further than their state associations for hand holding and encouragement to succeed. Various seminars and consultants are available with expanded services such as nutrition and rehabilitation. Success and change is just a will away. Will ya or won't ya should be the motto this coming year. Will ya follow my dream and see the bountiful results or will ya read into the negativity and maintain status quo. The choice is yours .. With the 4th, published in 1993, the Injury (Diagnosis-Related Estimates - DRE) model was introduced and served as the primary rating method. Most impairments were categorized on the basis of clinical findings resulting in a fixed impairment number for each category; surgery did not modify the original impairment. The 5th Edition, published in 2000, uses both the Diagnosis-Related Estimates (DRE) and the Range of Motion methods dependent on the condition. Categorization was based typically on findings at maximal medical improvement, definitions for the categories were revised, and four whole person permanent impairment values were provided for each category. The 6th Edition, released December 2007, uses a methodology based on the International Classification of Functioning, Disability and Health (ICF) and ratings are based primarily on a specific diagnosis, which results in assignment to an impairment class (IC), using grids designed for this purpose. The impairment value within a class is further refined by considering information related to functional status, physical examination findings, and the results of clinical testing. Range of motion is no longer used as a basis for defining impairment since current evidence does not support this as a reliable indicator of specific pathology or permanent functional status. The use of the "most recent Edition" will be required immediately by certain state jurisdictions, statute or code (AK, HI, KY, LA, MS, MT, NH, NM, ND, OH, OK, PA, RI, TN, VT, WY) and will be required for Federal and Longshore & Harbor Workers' Act cases.
