How am I doing? Ever have this question thrown at you after a few visits into a new patientıs care? As much as we pride ourselves on patient education, some seem always not to get it. Probably a good idea to step back a bit and realize we may be too deep in the process and therefore forget the mindset of a new patient. They are there to get well, reduce their level of pain and ultimately and hopefully to maintain their health and wellness. Their initial response to your questioning and probing was agreeable but after a few relaxed visits their level of curiosity has peaked. ³Doc, am I doing better and how can you tell?² Is your reply simply ³yes,² or do you discuss built in criteria taking into account both their subjective physical reactions and your objective findings? I hope so. Clearly, patients are there for a sole purpose. They will be responsive to your instructions if it makes sense and they either can feel results or visualize your plan and goals. A solid relationship begins after your handshake and supportive tone and smile. It continues with focused consultative questioning and further strengthened with your specific but unique examining style and report of findings. If your questions and decision making is clear, they should leave your office satisfied they are under proper care. The rest becomes routine and there, my friends, is the rub. Routine leads to complacency and complacency leads to poor practice management and judgment. Plugging patients into your office system without proper management usually results in excessive missed appointments with lost patients and revenue. Once a patient asks you how they are doing, be assured, they slipped through your educational void and the cracks are showing. You should have outlined their journey from the start and lead them clearly through it. Objective criteria, along with a blueprint of patient plans and goals, are crucial. Never keep it a secret. Your initial report of findings, should not only include a schedule and financial responsibilities, but also outline your plan and goals. Provide a written report, as elaborate and creative as you can but be specific why you arrived at your conclusions. Outline the specifics of their structural and soft tissue wear and tear at work and socially. Plot their accumulative physical affects of aging, such as accidents and other co-morbidity factors. Outline past and present spinal instability and predict the immediate future if changes are not forthcoming. If they work or play under the duress of repetitive activity, outline this and be specific how it is a negative, which is part of the cause resulting in their overall spinal effect. Be sure to stress this as it helps to stimulate referrals from their fellow work mates or sport partners. Visit by visit, especially during their first five visits, peak their curiosity with why and how your adjustments were made. Praise their success or chastise their failure to cooperate with your recommendations for exercise or daily living change. Stimulate referrals by having their spouse or significant other watch you adjust or best yet attend your office workshop so they too can be clear on your plan and goals. If you do this at minimum, your patient will never be in the dark, instead they will usually agree to maintain their care, and as far as practice and patient management goes, this is what itııs all about.
