Hi Doctor,
 
I can't recommend strongly enough to you that you NOT just use a 
copy of
someone else's contract.  For that reason, I don't provide them.  One 
of
the services I do provide is called the Successful Associate 
Program.
In that program I provide a questionnaire to both the senior doctor 
and
the associate to fill out.  
 
Once I analyze the questionnaires, I can give you advice on whether I
think you have the proper motivation and rationale/profitability to 
add
an associate, the areas where the two of you are compatible, the 
areas
where you should communicate/negotiate more, the areas where you 
have
red flags, and even - sometimes- a recommendation that you not 
work
together.  Sometimes I find that the associate doctor is being less 
than
honest about their stated goals.
 
Assuming that you are compatible enough to work together, I will 
then
give you a list of "ingredients" that should be in your contract based
on the goals, non-acceptables, relative level of commitment, and
especially expectations I elicit from the questionnaires.  This list
would be taken to a lawyer and included with the standard language 
and
clauses that are necessary for your state.  I have never done this 
with
a client that didn't feel I saved them time, money (esp. attorney 
fees),
and aggravation in excess of my $495 fee.  
 
Frankly, one of the criteria that tell me if you are ready to add an
associate is whether you are prepared to invest in securing the best
foundation for the success of the relationship.  Doctors who take 
this
decision too lightly (i.e. just take the first doctor who applies and
get a copy of somebody's contract from another state) inevitably 
find
themselves regretting ever getting involved with an
associate/independent contractor.
