Q: I am currently working at a clinic and ran into some problems. I suspect there is illegal stuff going on.......specifically billing for things that are not being done in the office. Also I found out they are getting there patients threw runners or a 3rd party who makes money off of the patients he brings in....This makes me want to leave, but I just feel bad leaving right out. It puts them in a compromising position. I was going tell them I am leaving and finish up my week there, but at the same time don't want to be associated with a clinic that does this because I puts my license on the line esp. now that I know about it. Not sure how to go about it; would it be in my best interest to just leave or should I finish out the week as a courtesy. Any help or insight on the matter would greatly be appreciated A: Thanks for the e-mail. This problem is not unique .. I've answered a number of these type questions over the years. Having a license to practice also makes us vulnerable to personal weaknesses .. such as doing some unethical things to make a buck. Runners may not be illegal but unethical. There are some states that have provisions within the board rules that prohibit this activity .. there are also some state attorney generals that use insurance fraud units looking for collusion between doctors (mostly DC's) attorneys and questionable cases. We all must make decisions on how we act .. I have always practiced conservatively and attempt to not be hooked into quick buck testing and practice growth procedures such as runners to make a living. If they charge for services not being performed this is fraud .. if your name is on the claims you are guilty .. if not .. you may be okay but the atmosphere stinks. So .. don't panic just be aware that you also must decide if the place you practice in is the place you want to be .. whether you make it difficult for you to leave them now or not. Start making plans to leave and walk away clean .. if they us runners then they are deep into it .. it is time to leave. Have a Great Day Dr. M
