and schedule another appointment. If, however, you have decided to 
discontinue care in our office care, please contact your primary care 

physician for an appropriate referral. As I indicated to you during 
our consultation, you do have some health issues that need to be 
addressed. If 

you find that you need chiropractic care in the future, you will 
certainly be welcome at our office. In the meantime, if I can be of 
any further 

assistance, please don't hesitate to contact me. I will be happy to 
help you in any way I can.
