It's a Dilemma I recently received a letter from a managed care health insurance contractor requesting I sign their agreement to accept a reduced fee for auto insurance patients that have coverage with a major auto carrier. Until now, auto insurance did not play the managed care game, at least not in my town but recently the cat seems be getting out of the bag. A few weeks later another contractor requested my signature to agree to accept auto insurance patients for a set fee of $50 per visit. I realize money is being squeezed everywhere and auto insurance carriers want to slim down their expenses but considering that PI patients may need and benefit with rehabilitative exercises, durable supports, a more extensive and progressive evaluations and most likely x-rays and let's not forget a few more adjustments than usual, a set $50 limit per visit prevents a simple business person, with compassion, to consider the time factor involved in processing these cases realizing that taking these patients cost more than it's worth. The insurance industry finally provided us with decent CPT EM, evaluation and management codes, that placed a value on the time needed to perform a service. Time is money and a fee structure was made a bit more liberal. Now this, a regressive trend that stifles our economic future and one in which we are asked to sign and agree to. What is a professional business person to do when their own bills are due, salaries must be paid, loans and others expenses will be coming from the same well and it is being dried by the paymasters, the insurance industry. I guess the answers harkens to the days of yore when cash was king and patients readily understood they were paying for their service. A cash practice takes more commitment, communication and compliance but even at this level, the energy expelled to create the ultimate cash practice, with insurance hovering in practices nearby, is difficult at best.
