Boxed In Chiropractic Recently I¼ve been reading a stream of e-mails from the same DC¼s preaching to ultimately place our profession in a box. Each of them have a similar mindset and profess a perception that they are holier than the majority of us as they are the guardians of truth and professionalism. Simply .. the majority of us are not scientific enough .. we have little respect from other healthcare providers .. we over utilize our services and worse yet they are extremely embarrassed to be amongst us mortals in the trenches trying to help others and make a living. Telling the chiropractic story is unprofessional .. marketing ideas to your community in an ethical and responsible manner is unprofessional. They feel that just a handful of visits .. let¼s say 4-6 .. should be enough to arrest a symptom and send the patients on their way. Nay to recommending supportive care. Managed care consultants drool over these recommendations .. requesting three to five visits over a years time. They toast each other for the chiropractic Skinner¼s box they helped create. What a profession .. without the armaments of diagnosis as found in typical medical settings or allied hospitals these DC¼s play with data and focus on minutia rather than the big picture. Symptoms are end results but where is the focus. Is it further testing and referrals or chiropractic care making the difference a difference. Patients come in various packages. During the 25 years of patients walking through my doors most have been to the medics first or have their family physicians in tow. Like Pavlos¼s dogs we salivate at the thought of being within insurance networks accepting their crumbs of money and bite our tongues when there are restrictions that go along with it. Don¼t get me wrong .. I am in many of these network programs and at my stage of practice life .. I wish to take it easy rather than do the hustle so I let the phone ring with little prodding but it comes with a price. The scientists within our profession should be applauded for their research with hard data and clinical trials. But chiropractic being chiropractic .. we treat .. we adjust .. we help healing .. it takes time .. often more time than insurance guidelines .. often above and beyond what Mercy guidelines dictate or actuarial charts recommend per code. Should we be boxed in because others are looking over our shoulders and may scream over utilization? Have you ever surveyed medical procedures or medication recommendations or physical therapy protocols? I think we under utilize .. under recommend .. under explain and get under paid for it. I hate to be boxed in and many of my colleagues hate it as well. They vary from Sherman to National graduates with all the divergent views in between. We are a physical discipline in the healthcare crowd and therefore need patient and insurance provider cooperation to stabilize and manage the problem and if patients desire further rehabilitation they should be offered such. There is no box and being that we live in a free society where freedom of choice and will reins .. I and you should provide a program that offers patients a chance to maintain spinal stability and flexibility. There is a chance that these are foreign terms to you and that relief care is all that is offered .. pain gone .. patient gone. Fine .. but for all others that have compassion and understanding that being a professional doesn¼t exist in a box .. tell your patients what you would tell your family. Periodic care is far superior to crisis care. Health and Wellness is not found in a Skinner¼s Box. And for the pious amongst us who feel so snobbish and above board .. shut up!
